Letter of Appreciation

Government of Washington, D.C., 2021

Mayor of Washington, D.C., Muriel Bowser, issued a letter of appreciation for the efforts of the nonprofit, Continue To Serve, in the aftermath of January 6th, 2021. Under my leadership, we organized over 350 veterans and civilians to walk through the streets, cleaning up the debris and destruction left behind by the violent insurrectionists that attacked the Capitol Building. Mayor Bowser acknowledged our efforts, covering over 2 square miles of downtown DC, removing over two truckloads of trash and debris, while at the same time, removing hundreds of fascist and racist stickers posted on street signs, walls, and local businesses.

Awarded a scholarship for my last semester of nursing school for my scholastic and clinical performance, based on the recommendations of my professors. Granted an internship at UMMC, Shock Trauma on the Multi-Trauma IMC with a guaranteed paid position upon graduation.

Navy and Marine Corps Achievement Medal

United States Navy, 2012

For professional achievement in the superior performance of his duties while serving as the Headquarters and Service (H&S) Medical Department Lead Petty Officer. In a time of need and lack of personnel, junior Petty Officer Third Class (E4) David Smith filled the senior leadership role of a Chief Petty Officer (E7) and effectively managed and coordinated the medical care of over 750 Marines and Sailors during the 2-1/2 month work up to deploy to Afghanistan. Petty Officer Smith increased medical readiness of the entire battalion from 60% to over 99% medical readiness, ensuring all personnel could conduct appropriate military operations during an 8 month deployment for Operation Enduring Freedom. Petty Officer Smith's personal initiative and steadfast devotion to duty reflected credit upon him and were in keeping with the highest traditions of the United States Naval Service.

Navy and Marine Corps Achievement Medal

For professional achievement in the superior performance of his duties while serving as Medical Department Head (MDR) for Navy Operational Support Center Decatur, Illinois. As MDR, he flawlessly managed the medical records and special cases for over 800 Navy reservists and 8 full time staff members, maintaining an exceptional 96% readiness rate. Additionally, he successfully identified, processed, and resolved numerous LOD and MRR cases, ensuring the effective and appropriate medical treatment of the sailor’s under his care. Petty Officer Smith's personal initiative and steadfast devotion to duty reflected credit upon him and were in keeping with the highest traditions of the United States Naval Service.
